Protocol to develop co-design activities

Guidelines to develop the co-design activities (national seminars, focus groups and world cafes) according to the target groups to be involved in pilot sites (end users – older people and families, health and social care practitioners and managers). The co-design is addressed to jointly develop a new outcome-based social and health integrated care model supported by a digital solutions.
